Program
The Patient
Assistance Pharmacy Program is a program offered to patients of any
physician. It is available to individuals with no insurance
prescription programs and who are of low income. Admission
criteria varies with each pharmaceutical company and is influenced
by income and the number of dependents in the
household.
Enrollment in
the program can occur in one of two ways: by physician
referral or by the patient contacting our office. Once the
paperwork is done, the forms are sent to the pharmaceutical company
for approval and processing, and upon approval the patient will
receive his medications in 4 to 6
weeks.
Our office is
located at 121 Watts Street, Suite E, in the same suite as
Kid-Med. The program is operated under the direction of Katina
Dark, PhTT, and our office hours are from 9:00 A.M. until 12:00 noon
Monday to Friday.
